Yes, I have placed my faith in Christ as my Lord and Savior. Through my path of spiritual formation I am increasingly aware of my sin and I grow to hate it more and more. My desires for righteousness and holiness and fellowship with God and man grow in place of my desire for my own glory.

Yes, I have accepted Jesus Christ as my Lord and Savior. I have prayed and ask God to forgive me of my sins and to come into my heart.

Yes. I received Christ into my life over twenty years ago now, there by surrendering myself to the Holy Spirit.

Yes, I am sure. I have recognized my sinfulness apart from Christ and my need for forgiveness of sins. I have believed (and do) that Jesus Christ is able to and has paid the penalty of my sin on the cross. Now, I have a “new relationship” with sin. I have been freed from its power and have recognized over the last several years how God is transforming my desires, attitudes, character, etc. by his Spirit.

Yes. When I was 7 years old, It was shared with me that I was separated from God’s Love because of the Sin in my life. If I wanted to be in heaven with Jesus, I must confess my sin, believe that Jesus is the Son of God and then invite Him to be Lord of my life.
